Certified Social Worker or RN Case Manager - Rehab - UofL Health

We are Hiring at UofL – Health Frazier Rehab Institute 

Location: 220 Abraham Flexner Way, Louisville, KY 40202

Shift: PRN 1st shift 

Our experienced team specializes in care and recovery for patients who have experienced brain injury, movement disorders, spinal cord injuries, stroke and other debilitating conditions. We provide services for individuals across the age spectrum – from pediatric to geriatric – and with the highest level of acuity, including patients who require mechanical ventilation.

Responsibilities

Job Summary

This position is responsible for providing comprehensive assessments on patients (pediatric and adult patients) and being the primary liaison between the patient / family and the treatment team.  The Case Manager ensures that the patient’s individualized treatment plan is carried out in a focused, cost effective, goal directed manner.  As a member of the interdisciplinary team the case manager will also facilitate the development, coordination, and implementation of the discharge plan as well as any other outside arrangements to ensure continuity of care. 

Qualifications

Job Requirements

• Bachelor’s Degree
• Five years minimum of case management experience in a rehab setting
• Kentucky licensed RN or Kentucky certified Social Worker

Preferred Requirements

• Master’s Degree
• Experience with patient/family counseling and problem resolution.
• Knowledge of Medicare, Medicaid, and Third-Party Payor systems
