The Phoenix Center for Rehab & Pediatrics is a skilled nursing facility located in Haskell, NJ caring for both the Pediatric and Geriatric populations. We are looking for a devoted and caring CertifiedOccupational Therapy Assistant to join our team on a per-diem basis.

Responsibilities:
- Collaborate daily with the Occupational Therapist Director and follow established plan of care for all patients
- Contribute to the resident’s assessment under supervision of the OTR
- Assist OT in identifying appropriate patients and referring them to rehab accordingly
- Assist OTR in developing treatment plans and techniques to implement the plan
- Implement and modify treatment plans under OTR supervision
- Document and maintain service-related records, in compliance with Medicare, Medicaid, HMO’s and departmental requirements
- Monitor resident’s response to treatment and modifies treatment during sessions in collaboration with OTR
- Report observations and residents responses to treatment to OTR and to other team members when so directed by the OTR
- Communicate and interact with other team members and the patient's family in collaboration with an OTR
- Perform any other related duties, as deemed necessary

Requirements:
- Degree from an accredited OTA program
- Actively licensed as an OTA in NJ
- Excellent verbal, written and interpersonal communication skills
- Results-oriented with strong critical thinking and problem-solving skills
- Positive attitude and willingness to be a team player
- SNF experience a plus but not required
- Occasional weekend and holiday rotation required
