Certified Occupational Therapy Assistant (COTA) - Phoenix Center for Rehabilitation and Pediatrics

The Phoenix Center for Rehab & Pediatrics is a skilled nursing facility located in Haskell, NJ caring for both the Pediatric and Geriatric populations. We are looking for a devoted and caring CertifiedOccupational Therapy Assistant to join our team on a per-diem basis.

Responsibilities:

• Collaborate daily with the Occupational Therapist Director and follow established plan of care for all patients
• Contribute to the resident’s assessment under supervision of the OTR
• Assist OT in identifying appropriate patients and referring them to rehab accordingly
• Assist OTR in developing treatment plans and techniques to implement the plan
• Implement and modify treatment plans under OTR supervision
• Document and maintain service-related records, in compliance with Medicare, Medicaid, HMO’s and departmental requirements
• Monitor resident’s response to treatment and modifies treatment during sessions in collaboration with OTR
•  Report observations and residents responses to treatment to OTR and to other team members when so directed by the OTR
• Communicate and interact with other team members and the patient's family in collaboration with an OTR
• Perform any other related duties, as deemed necessary

Requirements:

• Degree from an accredited OTA program
• Actively licensed as an OTA in NJ
• Excellent verbal, written and interpersonal communication skills
• Results-oriented with strong critical thinking and problem-solving skills
• Positive attitude and willingness to be a team player
• SNF experience a plus but not required
• Occasional weekend and holiday rotation required
