We partner with PACE (Program of All-Inclusive Care for the Elderly) to help these programs care for frail seniors. The PACE model is centered on the belief that it is better for the well-being of seniors with chronic care needs and their families to be served in their community whenever possible. 

CareLinx by Sharecare helps PACE programs with healthcare professional staffing and in-home care services including transportation to medical appointments, recreational activities, and other senior care services.

Job Summary:

CareLinx by Sharecare is looking for Per Diem Certified Nursing Assistants (CNAs) to join our team. 

The CNA provides services to PACE participants in adult day programming and in the home. As a caregiver, you will be responsible for providing various care services necessary for attending to the personal needs, support, and health status of participants in a home care setting. The CNA will provide supervision and support with clients as directed and report on any new conditions or needs that may arise. You will be making a difference in people’s lives and creating meaningful relationships with clients.

Essential Job Functions:

• Report on personal care services provided and the self-care abilities of the participant, including changes noted in the participant's condition
• Responsible for participant safety, identify safety hazards and emergency situations and initiate corrective action as able
• Provide behavioral supervision or support for participants, such as reality orientation, a reminder to follow through with nursing and medication instruction, diet restrictions, and emotional reassurance
• Assure compliance with infection control, personal protection equipment, blood-borne pathogens, hazardous material handling, and fire safety standards
• Maintain strict confidentiality of all participant information
• Prepare and submit timely written reports as required
• Maintain confidentiality of participant information
• Must be flexible to scheduling needs of participants. Evenings and weekends may be required

Qualifications:

• Active CNA license in California
• Current CPR certification
• Must possess, as a minimum, a GED or high school diploma
• Experience in a long-term care facility or with a community-based geriatric program preferred
• Minimum of six (6) months of documented experience working with frail elderly population highly preferred
• Knowledge of the principles and techniques involved in relating to frail elderly persons
• Skill in communicating with participants, their families, and other staff members; ability to establish a helping, trusting relationship with participants
• Demonstrates sensitivity to the needs of older adults as evidenced by a caring approach, patience in providing care, and responsiveness to participant requests
• Ability to follow directions, verbal and written instructions, and adapt work schedule under supervision
• Ability to utilize appropriate safety measures when providing care
• Ability to relate to the public and governmental agencies in a pleasant, patient, and professional manner
• Well organized, dependable, flexible, and resourceful
• Requires the use of a personal vehicle
