Key Responsibilities:

• Perform patient-centered care activities under supervision, ensuring safety and quality.
• Collaborate with the care team to meet patient needs and improve care processes.
• Document patient care, including hygiene, mobility, and vital signs.
• Identify and address patient concerns and risks effectively.
• Reinforce patient education to enhance care outcomes.

Qualifications:

• High school diploma or GED.
• Valid State of Wisconsin CNA Certification.
• BLS or CPR certification for healthcare providers.

Physical demands include lifting up to 50 pounds and exposure to various work environment hazards. Scheduled hours are 6:00 am - 2:30 pm, totaling 36 hours per week.
