Key Responsibilities:
- Perform patient-centered care activities under supervision, ensuring safety and quality.
- Collaborate with the care team to meet patient needs and improve care processes.
- Document patient care, including hygiene, mobility, and vital signs.
- Identify and address patient concerns and risks effectively.
- Reinforce patient education to enhance care outcomes.
Qualifications:
- High school diploma or GED.
- Valid State of Wisconsin CNA Certification.
- BLS or CPR certification for healthcare providers.
Physical demands include lifting up to 50 pounds and exposure to various work environment hazards. Scheduled hours are 6:00 am - 2:30 pm, totaling 36 hours per week.
