SUMMARY: Certified Nurse Practitioner (CNP) is responsible for providing medical services to the clients of The Crossroads Center. The CNP will provide psychotherapy and medical somatic services to the clients as directed by the Medical Director. The CNP will provide medication management care for those patients enrolled in Medication Assisted Treatment Program and provide support when needed to the Outpatient and Residential Services programs. The CNP is responsible for leadership and management of the dosing team.
ESSENTIAL RESPONSIBILTIES:
- Collaborates with Medical Director to provide; medical evaluation, mental health evaluation, medication review, crisis intervention, psychotherapy and ongoing treatment for clients.
- Ensures accuracy of diagnosis and appropriate pharmacologic interventions.
- Provide individual scheduled appointments for medication management, medication education; supportive psychotherapy and crisis interventions
- Works in conjunction with clinicians and nurses regarding mental health assessment, diagnosis, interpretation of psychiatric information and formulation and implementation of individual treatment plans
- Manages the nurses who comprise the dosing team for the MAT program.
- Utilize assessment information and acquired information to clarify problem areas and treatment needs, and establishes appropriate interventions.
- Participates as a collaborative team member in team meetings: maintains regular communications with team members and other providers to ensure coordinated services, and facilitates and participates in collateral meetings to enhance client supports.
- Assists in COVID testing for clients and team members.
- Document all clinical service activity and prepares progress reports as required by regulatory and funding bodies. Maintain case records and statistical data required for management information, billing and program evaluation
- Maintain open communication with all staff members.
- Maintain confidentiality regarding client and program information.
- Document relevant information in The Crossroads Center's Electronic Health Record and E-prescription software.
- Provides coverage for other CNPs when needed in Outpatient and Residential Services which may include preparing histories and physicals, review and medication management and other interventions as needed.
- Maintain professional conduct and boundaries.
- Other duties as required.
QUALIFICATIONS & REQUIREMENTS: To perform this work successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required.
EDUCATION & EXPERIENCE: Nursing Degree with licensure as a Nurse Practitioner by the State of Ohio, certification/experience with Behavioral Health Experience a plus. 1+ years’ experience treating adults.
CERTIFICATES, LICENSES, REGISTRATIONS: Licensure as a Nurse Practitioner by the State of Ohio, Buprenorphine Certified.
LANGUAGE SKILLS: Ability to read, analyze and interpret human service periodicals, professional journals, technical procedures, or government regulations. Ability to write reports, business correspondence, and procedure manuals. Ability to effectively present information and respond to questions from groups of managers, clients, customers and the general public.
REASONING ABILITY: Ability to define problems, establishes facts, and draw valid conclusions.
PHYSICAL DEMANDS: While performing the duties of this job, the employee is regularly required to talk, hear, stand, walk, sit, use hand to finger, handle or feel objects, tools, or controls, and reach with hands and arms. The employee frequently lifts or moves up to 10 pounds. Specific vision abilities required by this job include close vision and the ability to adjust focus.
