A Certified Nurse Assistant functions in a supportive relationship to professional disciplines and may function as an extension of these professional services with appropriate supervision and guidance assisting in the care of individuals in the long term care and assisted living/developmentally disabled facilities. Under the supervision of the nurse, or other professional personnel, performs personal care procedures, health related tasks, and other homemaking and housekeeping activities to maintain or achieve the patient's optimal level of health. Serves as a mentor to new Aides.
A Home Health Aide assists in providing quality patient care by performing assigned duties, health care tasks, and specified patient care activities according to the policies and procedures of Hospice Buffalo, Inc. and under the supervision of a licensed practical nurse and/or registered nurse. Assists in establishing and maintaining good interpersonal relationships with hospice patients, families, health care personnel, and the entire hospice staff and volunteers.
Essential Duties and Responsibilities for a Certified Nurse Assistant / Home Health Aide:
Assists with appropriate hygiene and grooming inclusive of bathing, hair shampoo, nail & skin care; oral hygiene; toileting/assistance with elimination; taking vital signs, and weights. Assists with health related tasks and personal care procedures.
Assists with activities of daily living.
Feeding the patient if necessary. Understands adequate nutrition and fluid intake.
Demonstrates effective observation and reporting skills. Maintains high level of confidentiality in compliance with HIPPA law and policy. Identifies and reports patient clinical changes and/or body function changes appropriately to RN Case Manager.
Demonstrates appropriate hand hygiene; waste disposal; use of Personal Protective Equipment; Bag (supply/tote) technique and knowledge of Standard Precautions, inclusive of basic infection control procedures.
Appropriately explains duties and functions of the Interdisciplinary Team including the role of the Certified Nurse Assistant.
Documents to agency standards.
Displays the ability to establish priorities and organize work and time appropriately. Anticipates problems and modifies plans and procedures to provide solutions.
Adheres to requirements of policies and procedures that are both patient focused (clinical) and organizational focused (administrative). Maintains the security of patient records.
Elicited no founded patient/family complaints, staff complaints or unusual occurrences (i.e., reportable incidents).
Demonstrates ability to communicate with patients and families in a supportive manner that is understandable and culturally appropriate.
Maintains a clean, safe environment; provides assistance with safe transfer techniques, positioning and ambulation. Demonstrates normal range of motion.
Able to read and record temperature, pulse, and respiration.
Recognizes emergencies and is knowledgeable in applying emergency procedures.
Is respectful of the patient, his or her privacy, and his or her property.
Qualifications for a Certified Nurse Assistant / Home Health Aide:
Education/Certification
Completion of a NY State approved Certified Nurse Aide training program with CNA Certificate and/or Home Health Care training program with a Home Health Aide Certificate.
High school diploma, GED preferred.
Certification in Hospice and Palliative Care encouraged.
Must have current Driver's License, free of any serious violations, and vehicle required for position.
Experience
One (1) year of experience in a clinical setting and nursing assistant experience preferred.
