Responsibilities:
Diabetes Care and Education Specialist to serve by providing care and diabetes self-care management education and support to diabetes patients in the Division of Endocrinology, Diabetes and Metabolism within the Department of Internal Medicine according to Medical Center policies and procedures:
- Conducts comprehensive diabetes assessments and develops care plans
- Provides personalized diabetes education to patients
- Coordinates care with referring providers
- Develops Diabetes Self-Management Education (DSME) through books, power point presentations and patient education handouts
- Provides nutritional and physical activity guidance; under physician supervision provides education and guidance for patients on medications, Self-Blood Glucose Monitoring (SMBG) and Continuous Glucose Monitoring (CGM) and diabetes technologies including insulin delivery device including insulin pumps, In-Pen and patch insulin devices
- Serves as diabetes expert to the medical and hospital staff
- Serves as integral member of the ADA Recognition education committee
Bachelor's Degree Required.
Registered Dietitian, Registered Nurse or Registered Pharmacist required; current licensure in the State of Ohio required.
- At least 2 years of work experience in a diabetes specialty and or patient education.
Preferences:
- Certified Diabetes Care and Education certificate.
- Ability to teach in settings with wide learner needs.
- Computer literacy with knowledge of electronic medical records (EMR) and Microsoft Office software applications desired
