Title: Caseworker
Position Type: Long-term, temporary (1+ years)

The Caseworker will provide all casework activities and services. Caseworkers will engage in daily activities with program management and Migrant Program leadership teams. The Caseworker will provide onsite or remote services by completing assessments, and exit services plans, and provide support with travel coordination and arrangements to assist asylum seekers transitioning out of the city's supportive housing and resettlement services programs.
- Conduct comprehensive assessments, develop exit service plans, and assist in coordinating travel arrangements to aid asylum seekers in their transition from the city's supportive housing and resettlement services.
- Reach out to family members, relatives, or other contacts to facilitate travel coordination.
- Deliver resettlement services beyond the confines of the city's supportive housing.
- Make referrals to community-based programs and organizations as needed.
- Record case notes, data, and information within the NY Health + Hospitals case management system.
- Collaborate with interpretation and translation services to ensure effective communication.
- Facilitate referrals to immigration legal services.
- Undertake additional tasks as required to streamline the provision of essential services to asylum seekers.
- Carry out any other job-related responsibilities as assigned.
- Bachelor's degree or a minimum of one (1) year relevant experience working in social services with vulnerable populations conducting assessments and creating service plans.
- Basic understanding of immigration laws, regulations, procedures, and benefits/entitlements.
- The ability to work collaboratively with families, collaborating agencies, and community professionals.
- Bilingual in Spanish or French (Must Have); Fulani, Arabic, Wolof (Nice to Have)
- Proficient in utilizing Microsoft Excel, PowerPoint, Word, and Outlook.
- Excellent verbal and written communication skills and ability to learn quickly and problem-solve under pressure.
- Flexible to potential shift changes based on operational needs.
