About the Job
Our client is seeking compassionate and detail-oriented Case Managers to join their team. In this role, you'll serve as a vital link between clients and resources, ensuring elderly and challenged individuals receive the care and support they need to maintain their independence and quality of life.

Responsibilities:
- Manage a dedicated caseload of clients, providing comprehensive care coordination and support services in set territories in the CSRA.
- Conduct in-home assessments and face-to-face interviews with clients in various settings
- Develop and coordinate individualized care plans in collaboration with clients, families, and service providers
- Advocate for elderly and challenged populations while protecting client confidentiality
- Monitor and document client progress, physical status, and service satisfaction
- Identify and coordinate appropriate community resources and referrals
- Report suspected cases of abuse, neglect, or exploitation to protective services
- Collaborate with healthcare professionals, including registered nurses, for medically complex cases
- Participate in community outreach initiatives and required staff training programs
Working hours: 8:00 AM - 5:00 PM
Skills:
- Experience working with elderly or challenged populations
- Knowledge of medical terminology and common diagnoses
- Strong interpersonal skills and ability to work with diverse populations
- Excellent organizational and time management abilities
- Ability to work independently while maintaining strong team collaboration
Education:
Bachelors
Experience:
1-4 years
Qualifications:
- Bachelor's degree in Social Work, Gerontology, or related field
- Relevant experience in case management and healthcare coordination; or adjacent fields
- Valid driver's license and ability to travel for client visits
- Proficiency with computer systems and electronic documentation
- Knowledge of Medicare, Medicaid, and Social Security programs
- Understanding of federal and state aging services guidelines
- Excellent communication and problem-solving skills
