About the Job
The Case Management Workflow Lead is responsible for developing and implementing patient care plans in collaboration with Discharge Planners (DCP), physicians, nurses, and the multidisciplinary healthcare team. Key responsibilities include:
- Leading and supporting DCPs through guidance, training, and participation in Multidisciplinary Rounds.
- Assessing patients' clinical and psychosocial needs and facilitating referrals to appropriate services.
- Communicating with patients, families, and healthcare team members regarding care plans and progress.
- Ensuring compliance with policies and standards while managing resources effectively.
- Collecting and analyzing clinical metrics and addressing variances in care plans.
Requirements include a Bachelor's in Nursing or a Master's in Social Work, four years of acute care case management experience, and relevant certifications. The position is full-time, based in Alexandria, VA, with a salary range of $79,664.00 - $129,438.40 per year and various benefits.
