Case Manager (tues-sat) - Inc.

We are seeking a dedicated Case Manager to join our team in Westborough, MA. This full-time position offers a competitive salary of $21 per hour, working Tuesday through Saturday. The ideal candidate will provide support and guidance to clients, ensuring their needs are met effectively.Responsibilities:

Assess client needs and develop personalized care plans

Coordinate services and resources for clients

Monitor client progress and adjust plans as necessary

Maintain accurate documentation and case records

Collaborate with community organizations and stakeholders

Requirements:

Bachelor's degree in social work or related field

Strong communication and interpersonal skills

Experience in case management preferred

Ability to work independently and as part of a team

Knowledge of local resources and services
