About The Position
**Job Title:** Registered Nurse - Case Manager **Job Specialty:** Case Management **Job Duration:** 13 weeks **Shift:** Day 5x8-Hour (08:00 - 16:30) **Guaranteed Hours:** **Experience:** Minimum of 2 supervisory references covering 1 year of employment within the last three years in a comparable specialty/setting **License:** **Certifications:** **Must-Have:** - Ability to float between all facilities within a given state - Completion of orientation and modules **Job Description:** - Work approximately 1-2 weekend days per month in exchange for a day off during the week. - Participate in the development of treatment plans. - Organize and participate in family meetings. - Coordinate care for patients experiencing homelessness, mental health crises, or with a history of substance abuse. - Independently arrange durable medical equipment, skilled nursing facilities, and ambulance transport. - Collaborate on admission criteria and protective services processes, such as InterQual. - Familiarity with drug/alcohol abuse treatment options, advance directives, and POLST. - Understanding of adoption and foster care processes. - Experience with HMOs, IPAs, and other healthcare organizations. - Commitment to working at least 3 of the 5 major holidays: Thanksgiving, Xmas Eve, Xmas Day, NYE, NYD. - Completion of non-billable modules prior to start and annually. All candidates are expected to fulfill all the responsibilities and maintain compliance with the travel guidelines provided.
