A DAY IN THE LIFE
As a Case Manager, every day is a little different, but your goal is always the same. Discerning the disabilities and accompanied needs of our participants as well as helping them achieve their goals is at the top of your priority list. You carefully make a plan for each participant and modify it annually or when there is a significant medical or social change.
With close attention to detail and professionalism, you communicate with other individuals and entities to ensure seamless coordination between physical, behavioral, and support services. In addition to managing cases, you are consistently developing and maintaining professional working relationships with participants and their families. You find great fulfillment in knowing that the work you do significantly improves the lives of others!
QUALIFICATIONS FOR A CASE MANAGER
BS/BA degree in related course work
Minimum of 3 years related experience providing case management for individuals with Intellectual Disabilities, Autism Spectrum Disorders, or Physical Disabilities.
Experience with Microsoft Office, state-maintained databases.
Must maintain proficiency in company sponsored training and certifications.
Maintain updated state related Clearances.
Successfully complete and maintain training courses as required or amended by program regulations.
Valid Driver's License including personal vehicle insurance coverage for liability and current vehicle registration, copies must be maintained on file with Care Lync.
