Case Manager, Rochdale Village Senior Center at Rochdale Village, Inc.

Duties and Responsibilities:

• Providing case assistance;
• Data entry for the S.T.A.R.S program;
• Facilitation of group activities;
• Provide necessary information and process referrals for supportive services;
• Complete applications and follow up to ensure clients receive assistance and services to meet their individual needs.

Qualifications:

• Bachelor's Degree (Required);
• Experience with client intake and conducting assessments;
• Ability to work with individual clients to identify their service needs and preferences;
• Good communication skills;
• Knowledge about eligibility for senior benefits and entitlement.
