• Hackensack Meridian Pascack Valley Medical Center is a 128-bed, full-service, acute-care community hospital with a new emergency department, a state-of-the-art maternity center, a women's imaging center, and an ICU.

• The RN Case Manager collaborates with Nurse Managers and Nursing Staff to coordinate patient care activities to ensure that care meets evidence-based practice standards or protocols and regulatory and payer requirements.
• The role integrates and coordinates utilization management, care coordination and discharge planning functions. Case Managers may work in the Emergency Department, inpatient units, or any other area within the hospital as deemed appropriate by the leadership.
• In addition to the job responsibilities of the Case Manager, this individual will be responsible for utilizing and training on the McKesson Medical Record System.

Job Requirements:

• Graduate of an NLN/AACN Accredited Program in Nursing.
• RN with at least two years' experience in Case Management.
• Experience with electronic medical records
• Current New Jersey State Professional Registered Nurse license.

Preferred Job Requirements:

• Case Management Certification a plus.
