ID: 50310206 Shift: Day 5x8-Hour (07:00 - 15:00) Description: This RN Care Manager would work on inpatient adult units such as medical/telemetry, orthopedic/spine, general surgery. We would try to place the RN Care Manager on a unit where they have expertise and keep them there as consistently as possible so they develop relationships with the care team and have continuity of assignment Will position float between units: Yes Is on-call required? No Are weekends required? Yes occasional weekends as needed Are block schedules required? No What are expected ratios? 1:17 Special requirements: assists with durable medical equipment, home health and multiple types of post-acute placements such as Skilled Nursing Facilities, adult foster homes, and assisted living. works with multiple community partners to arrange plans and secure authorization from insurance companies.
