Case Manager (RN). - Saint Francis Health System.

This position is ECB status, requiring a minimum number of worked hours per month with limited benefits. The Case Manager (RN) provides case management services, focusing on patient discharge and transition of care needs, and collaborates with the healthcare team to develop safe discharge plans to reduce hospital re-admissions.

• Minimum Education: Completion of an approved nursing curriculum.
• Licensure: Valid multi-state or Oklahoma RN License; CCM preferred.
• Experience: Minimum 3 years as an RN; case management experience preferred.
• Skills: Strong communication, organization, and clinical knowledge; proficiency in Microsoft 365.
• Responsibilities: Plan patient care, coordinate discharge processes, evaluate patient progress, and track re-admission reasons.
