About the Job

This position is ECB status, requiring a minimum number of worked hours per month with limited benefits. The Case Manager (RN) provides case management services, focusing on patient discharge and transition of care needs. Responsibilities include:

Collaborating with patients and the healthcare team to develop safe discharge plans.

Identifying barriers to discharge and coordinating planning processes.

Evaluating patient progress and obtaining necessary authorizations for post-acute care services.

Minimum qualifications include:

Completion of an approved nursing program and a valid RN license.

3 years of clinical experience, preferably in case management.

Key skills include strong communication, organization, and the ability to work independently. This role is located in Muskogee, Oklahoma.
