TPC is a a Certified Community Behavioral Health Clinic (CCBHC) and a community resource, providing all people with mental health and substance use services in their homes, schools, and neighborhoods.
TPC Case Manager and Residential Case Manager positions are eligible for $2500 sign on bonus and prorated for part time positions.
Job Summary:
Provide clinical case management services in order to facilitate program goals and objectives using a quality care approach.
Identify client needs with a person-centered approach to address not only mental health symptoms but co-occurring disorders.
Working with consumers to achieve the highest level of independence possible.
Requirements:
Residential Case Managers: High School Diploma or General Education Degree (GED); advanced education preferred.
Case Managers: Associate's degree in Human Services or related field required; Bachelor’s degree preferred.
Experience in residential facilities preferred.
Must have valid driver's license and insured auto.
Bilingual Spanish strongly preferred!
