The care coordinator will be a resource regarding services including but not limited to emergency financial relief, housing and eligibility/securing entitlements, health and wellness, and caregiver supports.ESSENTIAL FUNCTIONS:Core Catholic Charities Responsibilities:Exemplify Catholic Charities' mission, vision, guiding principles and policies and proceduresPromote adherence to high ethical standards, consistent with the Catholic Charities' Corporate Compliance Program, Code of Ethics, legal mandates & labor laws, NYS Justice Center, NYS, OMH, managing contractual regulations and other accreditation standards (e.g. COA, BBB)Promote and reinforce client-centered, strengths-based, solution-focused trauma-informed care in the context of systemsCultivate a culture of sensitivity and respect for diversityAdhere to all applicable federal and state laws and regulations including, but not limited to those governing confidentiality, privacy, program, billing and documentation standardsInfuses agency values in therapeutic care, offering quality, client-centered, strengths-based, solution-focused behavioral health and substance abuse services, as well as with collateral contactsPerform all duties in accordance with CCB's corporate compliance and ethics programParticipates in required staff meetings and designated trainingCore Job Responsibilities:Maintains active caseload of assigned clients and ensures all services are in compliance with contract requirements, and accreditation standardsMeet client in-home, community, or office, based on client preference and safetyActs as an advocate for clientsIntake & Assessment: Conduct comprehensive assessments including reviewing referral information, identifying and engaging key participants, identifying individual strengths and needs and the external factors impacting the clientCare Planning & Service linkage: Utilize person-centered practices to assist the client with the development of an Individualized Services Plan (ISP) addressing:Activities, services, supports, and providers that will help in recovery, wellness and satisfaction based upon the individual's interests, preferences, and strengthsSkills needed to achieve personal relationships, community inclusion, dignity, and respectSchedule and facilitate service plan reviews, including evaluation of advancements and barriers to treatment progress; revise the plan as appropriateMaintain clear and concise documentation of interventions that demonstrate compliance with the principles of interdisciplinary team as well as funder requirements and accreditation standardsProvide community education including but not limited to Powerful Tools for Caregivers and other evidence based curricula, to enhance public knowledge, caregiver support, outreach and public relationsEDUCATION: Associates degree in social work, mental health counseling, gerontology or other approved related field preferred. Bachelor's degree preferred.EXPERIENCE: Two (2) years direct experience linking adults age 60+ to a broad range of essential, community based services.KNOWLEDGE, SKILLS, and ABILITIES:Pleasant, welcoming, and professional demeanor requiredDemonstrated compassionate behavior toward older adults in need, particularly those with behavioral health diagnosesExceptional interpersonal and communication (oral and written) skills requiredBasic understanding of psychiatric and substance use disorders in older adultsIn-depth familiarity with community health, behavioral health and social services systems, organizations and resources.
