A little about the job....

• Is accountable for coordinating care delivery for an identified caseload of patients in collaboration with medical staff and other members of the interdisplinary team.
• Facilitates pre-certification and payer authorization processes; facilitates collaborative management of patient care across continuum, intervening as necessary. 
• Plans to meet patient needs to ensure high quality of patient care, reduced length of stay efficient utilization of resources and appropriate reimbursement of care and services. 
• Apply process improvement methodologies in evaluating outcomes of care. 
• Supports Clinical Documentation Management Program (CDMP) by performing medical record reviews, identifying correct DRG designation, ensuring documentation accurately reflects acuity. 
• Serves as clinical resource for medical records coders and coordinates communications with physicians.

Qualifications for the job... 

• Graduate from an accredited school of nursing and currently licensed by the State of Alabama, or Masters degree in social work from an accredited school of social work and currently licensed by State of Alabama or eligible for licensure. 
• Bachelor's preferred (nursing or associated field) CCM preferred. Currently licensed as RN by State of Alabama.    
•  Three to four years' experience with one year recent clinical experience in specialized area or one year clinical case management experience. 
• Utilization Review experience preferred.
