Job Details
We are seeking an enthusiastic and dedicated Case Manager (LVN) to join our dynamic team in the Engineering industry. As a Case Manager, you will be responsible for managing the health care services for our employees, ensuring their mental and physical health needs are met, and fostering a safe and healthy work environment. This role involves a great deal of multitasking, excellent communication skills, and the ability to work in a fast-paced environment. If you are an experienced Licensed Vocational Nurse with a background in case management and mental health, we would love to hear from you.
Responsibilities:
1. Provide comprehensive case management services to our employees, including health assessments, planning, implementation, coordination, monitoring, and evaluation.
2. Coordinate and manage health care services, ensuring that the necessary services are received at the right time and in the right setting.
3. Conduct regular mental health assessments and provide support and referrals as needed.
4. Collaborate with healthcare providers, family members, and other stakeholders to ensure the best possible care for our employees.
5. Facilitate communication between employees, healthcare providers, and management.
6. Develop and implement health and wellness programs to promote a healthy work environment.
7. Maintain accurate and up-to-date case files and documentation.
8. Ensure compliance with all relevant laws, regulations, and company policies.
Qualifications:
1. Must be a Licensed Vocational Nurse (LVN) with a valid license to practice.
2. Minimum of 3 years of experience in case management, preferably in an industrial or engineering setting.
3. Proven experience in mental health support and counseling.
4. Exceptional communication skills, both written and verbal, with the ability to effectively communicate with a diverse range of individuals.
5. Strong organizational skills, with the ability to manage multiple cases simultaneously and meet deadlines.
6. Proficient in using healthcare software and computer systems.
7. Knowledge of relevant healthcare laws and regulations.
8. Ability to work independently and as part of a team.
9. Compassionate and empathetic, with a strong desire to help others.
10. Excellent problem-solving skills, with the ability to think quickly and make decisions in a fast-paced environment.
