About the Job
- Provide a minimum of 115 hours of service for assigned clients each month.
- Refer incarcerated individuals to short-term residential treatment facilities.
- Complete client interview and submit written findings and recommendations to referral source
- Coordinate admission with residential treatment facilities and Monroe County Jail
- Coordinate services and provide the court with routine progress reports as required by the conditional release order.
- Coordinate diversion plans of individuals charged with felonies in the Monroe County Jail who are at risk of commitment to the Department of Children and Families, but who may be diverted to the community by the forensic team.
- Coordinate the provision of mental health treatment, competency restoration training, residential care or housing with supervision, medical and auxiliary services if appropriate, case management and monitoring of individuals who are being diverted from commitment to the Department of Children and Families.
- Attend court hearings, obtain conditional release orders and ensure individuals are monitored in the community in accordance with the terms of the conditional release order.
- Coordinate discharge planning activities for forensic clients at state treatment facilities
- Participate in treatment team, and discharge planning meetings for forensic clients at state treatment facilities.
- Develop and submit conditional release plans to state treatment facilities and to the committing court.
- Attend court hearings in the cases of individuals being discharged from the state treatment facilities and ensure effective linkage to service continuum.
- Ensure monitoring of individuals on conditional release order in Monroe County, including individuals transferred into the counties from other circuits.
- Ensure individuals on conditional release order and monitored in accordance with the court order and to ensure compliance with the order and department rules.
- Ensure committing court is immediately notified by phone and in writing of any deviations from the conditional release order
- Ensure review of required monthly monitoring reports in order to intervene in problematic situations, to provide alternative treatment modalities when necessary, and to identify trends and issues that illustrate opportunities for improvement in service delivery
- Should be CAP, CPRS, or CPMH eligible. If not certified, must have ability to obtain the credential within 2 years of employment Preference will be given to those candidates who are currently certified
- Must be CPR/First Aid certified within 6 months of employment and every two years thereafter.
- Graduation from an accredited four-year college with a degree in Addiction Counseling, Criminal Justice, Social Work, Psychology or related field is required
- Two years of related work experience in Mental Health, and / or Substance Abuse field preferred
- Ability to meet requirements of Monroe County Sheriffs Office background check
- CPR Certification, First Aid Certification and an annual Tuberculosis test.
- Ability to provide an Affidavit of Good Moral Character
- Clearance in Pre-Employment Drug Testing and Criminal Records Checks, as required.
