POSITION TITLE: Community Outreach Case Manager
DEPARTMENT/PROGRAM: Counseling
REPORTS TO: Clinical Supervisor
SCHEDULE: FULL TIME (Determined on a case by case basis)
GENERAL STATEMENT OF DUTIES: Increase access to mental health, addiction and empowerment programming within underserved communities.
ESSENTIAL RESPONSIBILITIES:
1. Provide case management services to children and adults within underserved communities.
2. Provide assistance to individuals in achieving personal independence, managing basic needs, and developing skills of daily living.
3. Assist individuals with accessing natural support systems in the community and/or linking them to formal community services.
4. Conduct advocacy and outreach within underserved communities to eliminate barriers to seeking or maintaining education and employment; behavior health services; physical health services; or other services as needed.
5. Coordinate with local providers to bring services to the center, including but not limited to: educational activities, mental health services, parenting/caregiver outreach services, suicide prevention programming and other program programming, preferably evidence based, targeting at risk and underserved individuals.
6. Transport participants as needed for program success.
7. Complete all necessary documentation, such as time sheets, case notes, and worksite review forms as grant/funding program requires in a timely manner.
8. Represent the agency/program to the public in a positive and energetic manner.
9. Comply with all grant requirements, including evaluation and ensuring objectives are met.
10. Complies with agency policies and procedures, COA regulations, federal and state requirements, and educational/certification/registry requirements.
11. Other duties as assigned.
QUALIFICATIONS:
1. Valid Ohio Driver's License and ability to meet agency requirements for driving.
2. Must be able to meet agency requirements for criminal back ground check (Ohio Bureau of Criminal Identification & Investigation).
3. Strong written and verbal communication skills, including ability to present, and the aptitude to develop effective relationships.
4. Strong computer skills.
MINIMUM EDUCATIONAL/LICENSURE REQUIREMENTS: BS or BA from accredited university preferred.
MINIMUM EXPERIENCE REQUIREMENTS: Social Service or mental health experience required. Experience with children required.
PHYSICAL REQUIREMENTS: Position will require transporting participants.
