About the Job
The Case Manager, Jobs Plus Program will coordinate case management services that include supportive services with grant partners and community providers, including: workshops, support groups, training programs, and activities for residents that promote self sufficiency by becoming gainfully employed with living wage jobs.
The essential duties of this role include:
- Assesses the social, economic and support service needs of residents and makes referrals for appropriate case management services.
- Plans, implements, monitors and coordinates all case management activities for education, employment and supportive services.
- Develops and maintains case plans and files, providing feedback relating to program activities, and updating case management activities into the appropriate database.
- Conducts evaluation of residents for job training and employable skills.
- Provides needs assessment and conceptualization that provide an opportunity for residents to reach goals towards educational and employment success.
- Provides advocacy and guidance to participants of the Jobs Plus program to assist with addressing residents' self-development, educational, employability and support service needs for selecting a career path.
- Cultivates resident leadership within the program, fostering community engagement and empowerment.
- Assists residents as they overcome barriers (childcare,transportation, etc.) to employment.
- Other duties as assigned.
Qualified appicants will have the following knowledge, skills and abilities:
- Bachelor's degree in human services, management, business administration or related field preferred.
- Experience in human services, community programs, community development, non-profits, or related fields.
- Strong organizational skills with the ability to manage multiple tasks and proirities effectively.
- Proven ability to coach, develop and empower others.
- Execllent interpersonal and communication skills, both written and verbal.
- Ability to work collaboratively in a team environment and build partnerships with diverse stakeholders.
- Must be able to start within 30 days.
