Description
SUMMARY
Under the supervision of the Director of Social Services, provides direct case management services to clients. The schedule for this position is: Tuesday, 12 noon-9 pm; Wednesday, Thursday, Friday and Saturday, 8 am-5 pm.
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties and responsibilities may be assigned.
+ Conducts an initial intake meeting within 24 hours of the client's arrival to the Sanctuary Program.
+ Determines clients’ needs by completing intake interviews; and ensures clients are orientated to the program guidelines.
+ Coordinate referrals for healthcare, mental health, and educational services.
+ Enhances the program, by maintaining community relationships with all providers.
+ Maintain and prepare clients’ records, by reviewing case notes.

EDUCATION
+ High School Diploma / GED Equivalent
+ Bachelor's degree in social work or human services field, preferred.

EXPERIENCE
+ Minimum two years in a behavioral health/substance use disorder treatment program.
WORK ENVIRONMENT
+ This job operates in an indoor office environment. This role routinely uses standard office equipment such as computers, phones, and photocopiers.
