Knoxville Center for Behavioral Medicine - a comprehensive inpatient behavioral health facility. Our center will provide acute behavioral health care and outpatient treatment to adults and geriatric adults who are in crisis from severe symptoms related to behavioral health and/or substance use disorder.

What you will be doing:
- ESSENTIAL FUNCTIONS:
- Assessment and Evaluation: Assist in conducting comprehensive assessments of clients' mental health, substance abuse, medical history, and psychosocial needs to develop individualized treatment plans.
- Treatment Planning: Collaborate with interdisciplinary teams to develop, implement, and review treatment plans tailored to clients' needs, goals, and progress.
- Case Coordination: Coordinate and monitor clients' care, including scheduling appointments, arranging transportation, and linking them with appropriate community resources and support services.
- Utilization Management: Complete all Medicare Outpatient Observation Notice (MOON) for Medicare clients. Complete any Coordination of Benefits with clients for insurance purposes.
- Referrals: Responsible for receiving and managing referrals within KCBM or outer agency. The duties include reviewing referral information, assessing client needs of intensive outpatient services based on the provided information, coordinating services or resources, and ensuring timely follow-up and communication with all parties involved. This is a crucial role in connecting clients with appropriate support and services, striving to enhance client outcomes and overall care coordination.
- Crisis Intervention: Provide immediate crisis intervention and support to clients experiencing acute mental health or substance abuse crises, including coordinating emergency services when necessary.
- Advocacy: Advocate for clients' rights, needs, and preferences within the treatment team, healthcare system, and community to ensure access to quality care and support.
- Education and Support: With the assistance of the Qualified Mental Health Professional (see IOP-1), case manager for IOP/PHP will help educate clients and their families, when needed, about mental health and substance abuse disorders, treatment options, coping skills, and relapse prevention strategies for encouragement for following discharge treatment recommendations.
- Documentation and Reporting: Maintain accurate and up-to-date documentation of client assessments, treatment plans, progress notes, and discharge summaries in compliance with legal and regulatory standards, as well as policy and procedure of KCBM.
- Paperwork/Information for Clients: Responsible for managing and distributing relevant paperwork to clients and ensuring it is readily available in the lobby. This includes forms for group therapy, community resources, and any certifications of completion. They will also oversee the printing and posting of the weekly group therapy schedule in the lobby to keep clients informed and facilitate their participation in scheduled sessions. The case manager is also responsible for providing the virtual group link to clients that are in virtual programming. With that, the case manager is responsible for completing the virtual HIPAA training by the director of intensive outpatient services and abiding by the training when completing this task.
- Collaboration and Communication: Communicate effectively with clients, families, treatment team members, healthcare providers, and community agencies to facilitate seamless care coordination and continuity of services.
- Monitoring and Follow-up: Monitor clients' progress throughout the treatment process, conduct regular follow-up assessments when needed, and adjust treatment plans as needed to address evolving needs and goals.
- Self-Care and Professional Development: Engage in ongoing self-care activities and professional development opportunities to enhance skills, knowledge, and resilience in managing the challenges of the case manager role.
- Ethical and Legal Compliance: Adhere to professional ethics, confidentiality regulations, and legal requirements in all aspects of case management practice, including maintaining client privacy and confidentiality.
- Company and Legal Compliance: Comply with organizational policies, procedures, performance improvement initiatives and maintains organizational and industry policies regarding confidentiality.
- Complete other duties as needed, instructed by the Director of Intensive Outpatient Services.
EDUCATION/EXPERIENCE/SKILL REQUIREMENTS:
High school diploma or equivalent or bachelor's degree preferred.
LICENSES/DESIGNATIONS/CERTIFICATIONS:
First Aid, CPR, require a valid state driver's license where transporting person(s) receiving services is an essential function.
OTHER FUNCTIONS:
