We are currently seeking a Targeted Case Manager to work in our Integrated Health Care Clinic in Syracuse, NY. 

Position Summary: 

The Targeted Case Manager coordinates care for clients who are receiving services from the clinic and would benefit from case management services and support. 

Job Responsibilities:

• Works with clients to determine immediate and ongoing need for case management services; assists eligible clients with Health Home enrollment 
• Conducts ongoing assessments to identify individual’s strengths, and barriers, and establish clear goals to improve the overall health and wellness of clients, this includes linking individuals to additional supportive services as needed.  
• Coordinates and collaborates with client’s care team in the development of a comprehensive Plan of Care; ensures the plan is shared 
• Assists as needed with the collection of Quality of Care monitoring 
• Maintains relationships with Community Service providers, Physicians, Specialists, Mental Health providers, Hospitals and other services to ensure individuals are well supported. Establishes clear lines of communication to ensure the best quality of care is received.  
• Completes baseline assessments for individuals enrolled in the clinic as needed; monitor reassessment due dates and ensure those are completed either 30 days prior to the due date or 30 days after the due date; complete clinical discharge assessments.
• Completes necessary documentation including assessment, progress notes, and care plans as applicable and in accordance with program and agency standards.  
• Assists and advocates for clients in applying for various financial benefits, such as SSI, SSD, Medicaid, Public Assistance, ACCESS-VR, VA, and other public and private funds. 
• Provides individualized services in a manner which is culturally competent. Empowers clients by using clients’ cultural strengths and informal support networks in service delivery. 
• Provides door to door transportation as required.  Ensures safety and welfare of passengers at all times.  Makes us of personal vehicle for program transportation as required.  
• Maintains effective professional relationships with other service providers and community organizations in the interest of clients’ and program goals in accordance with the Agency Policies and procedures. 
• Performs other duties, responsibilities and related tasks as assigned. 

Qualifications:  

Bachelor’s degree in related field or Associates and one (1) year experience or more. Must have a valid New York State driver’s license and access to reliable transportation.  Demonstrates the values of diversity, equity and belonging; foster an inclusive environment that facilitates diversity.

Pay range: $20/hour to $22/hour.
