About the Job
The Service Coordinator 2 is responsible for planning, scheduling, staffing, coordinating, and monitoring services for participants. Identify, initiate and implements quality changes to benefit the program. Ensures that staff treats people with respect and listen carefully to them to best honor their choices, privacy, refusals and rituals while also providing appropriate safeguards. Assists people to identify and resolve any needs or concerns they may have. Assists people to learn about and advocate for their personal rights and to understand the responsibilities that accompany those rights. Ensures that all required regulations are met. Ensures that staff supports people with the kind, amount and style of assistance they need to enjoy personal well being. Recognizes that people's need for support changes as their life circumstances change and adjust supports as needed.
Responsibilities:Develops and ensures the implementation of Individualized Support Agreements for program participants. Develops goals, objectives, strategies, and guidelines based upon the needs of the individual, as determined by the Interdisciplinary Team as indicated by the person’s anniversary date.
Coordinates, facilitates and implements the agreed upon services in a person’s Support Agreement, ISP and Staff Team Meetings and communicates these services effectively to all stake holders.
Monitors program goals and objectives to ensure alignment with agency mission and strategic plan.
Develops and maintains an annual calendar of all scheduled responsibilities.
Provides and/or obtains counseling, guidance and education to program participants as necessary. Coordinates all ancillary services and activities provided within the agency and in the community (Psychology, Therapies, etc.).
Maintains all case records according to professional standards, HIPAA, and funding source guidelines and accrediting organizations.
Liaisons and State agencies, others preferred, family members and other involved parties/agencies to ensure quality care.
Completes performance reviews for employees. Coaches and counsels employees regarding job performance as needed. Provides personnel support services by interviewing, hiring and orienting new employees. Monitors training for supported staff. Must be knowledgeable of and adhere to the union contract in managing direct support staff.
Develops and maintains a staff schedule to ensure that sufficient qualified staff are available to meet programmatic needs.
Attends to time sensitive material.
Assists participants to navigate the Seven Hills Rhode Island system as well as other service delivery systems.
Reports urgent, important issues regarding service delivery system to supervisor.
Reports inability to perform any part of duties to supervisor and participates in a plan for improvement.
Develops budget and ensures program expenditures fall within projected budget.
Has a working knowledge of all jobs under his/her supervision. Provides direct support services to include lifting and therapeutic holds as necessary.
Fosters relationships with families, friends, etc.
Understands and practices principles of incident management process.
Follows doctors’ orders and ensures that medications and treatments are administered following proper medical procedures. Monitors the dietary needs of the program participant. Recognizes, reports, and assists with medical crises as they occur.
Requires a Degree in Human Services or a related field, three years of progressive experience in human services with at least one year of management experience preferably working with adults with developmental disabilities, or equivalent combination of education and experience.
Must possess computer skills and the ability to navigate a network system.
