Job Summary:
The Providence Center ICT Latino Team is looking for a fluent English & Spanish bilingual Case Manager to work with a multi-disciplinary team which consists of case managers, substance abuse specialists, psychiatric registered nurses, clinical therapist and a psychiatrist to coordinate services for consumers to minimize stressors, provide psychotherapy and to facilitate the reduction of psychiatric symptoms in an individual and/or group setting.
Duties and Responsibilities:
Coordinate services for consumers to minimize stressors to facilitate the reduction of psychiatric symptoms.
Provide support counseling, problem solving, contracts and limit setting.
Participate as a member of a multi-disciplinary team in the development and implementation of therapeutic services, to include working with substance abusers, as it relates to case management.
Make assessments and provide appropriate crisis interventions and risk management as needed.
Teach consumers adult daily living skills.
Initiate treatment and advocate for consumer within network of care.
Negotiate on behalf of the consumer, as needed, with landlords, courts, families, the treatment team, and consumers themselves.
Transport consumers in own vehicle or TPC vehicle to planned activities and appointments.
Complete recordkeeping in accordance with Health Information Services (HIS) and other center requirements within required time frames and abide by center confidentiality policy.
Maintain cooperative relationships with TPC staff, consumers, community agencies and the public.
Attend trainings, case presentations and conferences.
Other duties as assigned by Team Manager or Director.
Requirements:
Associate's degree required, Bachelor's degree in Human Services or related fields preferred.
Experience in Human Services field with psychiatrically disabled population preferred.
Fluency in Spanish/English (reading/writing/speaking) required.
Ability to read and interpret instructions, documents and follow procedures. Ability to write routine reports and correspondence. Ability to speak effectively with consumers, employees, and individuals in the community.
Must have valid driver's license and insured auto.
Knowledge of basic crisis intervention, counseling techniques, and case management techniques is preferred.
Ability to act as an advocate for the needs of the Hispanic/Latino community is required.
