Position Title: Case Manager I
Department: Certified Community Behavioral Health Clinic (CCBHC)
Reports To: CCBHC Clinical Supervisor and Senior Supervisor
POSITION SUMMARY
The Case Manager I position with CCBHC involves screening for eligibility criteria and providing comprehensive case management services to patients with co-occurring Substance Use and Behavioral Health diagnoses. The role includes coordinating care, offering primary care assistance, creating individualized care plans, and tracking patient progress from diagnosis to treatment completion. In addition as a Case Manager I you will also be responsible for linking patients to preventative care resources and ensuring linkage is complete.
SALARY RANGE:
$25.75 - $29.50 per hour DOE

8 per Day / 5 Days per Week
CATEGORIES OF DUTIES
General Duties:
1. Conducts self in a professional manner in areas of dress, language, attitude and behavior.
2. Exhibits full knowledge and support of the philosophy and objectives of the Agency.
3. Fully understands and maintains policies regarding professional ethics.
4. Inform patients about other services offered by Tarzana Treatment Centers, Inc. (TTC) that may be appropriate for the patient’s family members.
5. Attends in-services trainings and educational workshops.
6. Attends all required staff meetings.
7. Cooperates and communicates fully with other staff members regarding patient behavior, progress activities, etc.
8. As appropriate works closely with other multi-disciplinary team members to develop a comprehensive Individualized Treatment Plan on assigned patients and to ensure that all identified problems are addressed.
9. Assist with employment/vocational referrals, provide vocational counseling re resume preparation, job interviewing etc.
10.Provides outreach and follow up services to patients in and out of treatment, to check progress, offer additional services, help reengage in treatment, make needed referrals.
11.Facilitates discharge-planning efforts, commencing at time of admission on all assigned patients, including areas of housing, employment, medical psychiatric, financial, aftercare, etc.
12. Makes direct contact within agency and with outside entities as appropriate, in order to assist patients in resolution of legal, financial, housing, medical, psychiatric and discharge planning issues
13.Provides regular, comprehensive progress note charting in patient’s Medical Record which document contacts, progress towards identified goals, program participation, referrals, discharge planning and other case management activities.
14. Performs other duties as assigned by Director and/or Administrator.
EXPERIENCE
1. For AOD Programs:
Case Manager I
- Enrollment in an academic Addiction Studies program and registration with CCAPP, CAADE, or CADTP.
- 1 year experience in a Behavioral Healthcare setting.
2. For all other Programs:
Case Manager I
- 1 year experience in a Behavioral Healthcare setting (HIV programs) and DVSS Certification for DV specific staff
3. Bilingual English/Spanish preferred
4. Valid California driver’s license and insurance
LICENSES/CERTIFICATES
For AOD programs, registration or certification with a DHCS approved state level counselor certification agencies.
