Case Manager - Housing Specialist
The Case Manager - Housing Specialist is integral in assisting clients at One Community Health towards achieving housing stability, and healthy, self-sufficient living. This role involves direct client services, housing assistance, collaborative efforts with local organizations, and administrative responsibilities. The ideal candidate will have a strong background in social work, with a profound understanding of housing issues and resources in Sacramento County.
This role is on-site in Sacramento, CA (95811).
Essential Functions
- Offer direct assistance in housing searches, developing and maintaining an inventory of appropriate housing units for clients and their families, ensuring access to suitable options.
- Educate clients on how to secure and maintain permanent affordable housing, including understanding tenant/landlord rights and responsibilities, to promote temporary or long-term housing stability.
- Work closely with local agencies and landlords to locate and secure housing, connecting clients to supportive housing programs that meet their specific needs.
- Provide case management services to assigned participants, guiding them towards achieving housing stability.
- Document all efforts and activities in relevant electronic systems, maintaining accurate files on client and property/landlord relationships. Additionally, provide training and technical support on housing resources to both internal staff and external partners.
- Ensure all client data is accurately entered into the Homeless Management Information System (HMIS) and other necessary databases, supporting efficient case management and program reporting.
Additional Duties
- Participate actively in meetings, case conferences, and training sessions to foster professional growth and development, ensuring up-to-date knowledge and skills in the field of HIV/Gender Affirming Care and patient management.
- Conduct community engagement activities, which include managing information tables, delivering presentations, and planning events to raise awareness and educate the community about housing services.
Minimum Requirements
- Highschool diploma or GED
- 3 year's related work experience.
- Valid BLS Certificate
- Must have a valid CA drivers license and adequate insurance on registered vehicle. Field testing is required for this position.
Preferred
- Bachelor's degree in Social Services or related field
- 1 year of experience providing case management
- 1 year of experience providing services amongst LGBTQ, minority/underserved communities, incarcerated and unhoused populations.
- Experience with electronic medical records
