Position Details



The Case Manager is responsible for assessing, planning, and skill transferring recovery treatment and support plans for adults and youth with an Intellectual Developmental Disability and mental health diagnosis. They provide intensive therapeutic services in all types of settings, including but not limited to community living arrangements, schools, community settings, natural support residents, etc. The Case Manager, in collaboration with other members of the treatment team, implements and transfers skills identified in the recovery treatment and support plan that utilizes a person-centered approach.

The ideal candidate will possess the following qualifications:


* Bachelor’s degree in Social Work, Psychology, or another human service-related field required.
* At least one (1) year of experience in the behavioral health field required.
* Three (3) years of experience in a mental health setting and/or one year working in a developmental disability setting preferred.
* Possesses or can obtain all necessary clearances and background checks to work with designated consumers required.
* Valid PA driver’s license with a clean driving record required.
