Your role as a Case Manager
As a Case Manager in our Payson Family Clinic, you will play a crucial role in providing comprehensive case management services to our clients. By evaluating client strengths, resources, and needs, you will assist individuals in achieving their personal goals. You will conduct thorough assessments related to various aspects of their lives, including legal, medical, housing, and mental health needs. Coordinating with family members, landlords, and other agencies, you will ensure that clients receive the support and services they require to thrive. Your advocacy may extend to court hearings and treatment team meetings, where your expertise will be invaluable.
Schedule: Mon-Fri 8am-5pm OR 9am to 6pm

Knowledge and skills required for the position are:
- Case Manager II Minimum Qualifications:
- Graduation from a college or university with a Bachelor's degree AND Licensed as a Social Service Worker (SSW).
- Case Manager I Minimum Qualifications:
- Graduation from a college or university with a Bachelor's degree in social work and working towards SSW licensure;
- OR Graduation with a Bachelor's degree in any field and the applicant has completed coursework required by Division of Occupational and Professional Licensing to apply for SSW licensure per Utah Code 58-60-205:
- Human growth and development; Social work practice and methods
- SUDC and ASUDC applicants also accepted.
- Spanish speaking preferred but not required.
