Case Manager - ESOL and Workforce - Catholic Charities Community Services

 The International Center provides educational programming for newly arrived immigrates and refugees with the goal of improving learners English proficiency and communication skills. The Case Manager will work with students enrolled in the Bridge to Healthcare Program. This program will identify and recruit qualified adult students who are interested in careers in healthcare and related fields and offers access to an enhanced training program as well as placement with employers upon graduation. Under the supervision of the Project Manager, the Case Manager will provide individual support to the students enrolled in ESOL and workforce programming to help address social service needs that might be barriers to persisting in the program. This position will also support the operational and administrative needs of the program.

Essential Duties and Responsibilities include the following. Other duties may be assigned.

o Oversee cases including screening, assessment, intake, enrollment, ongoing case management, and case closure or transition.

o Provide social services, make referrals, and advocate for and accompany clients as needed to advance their cases and meet resettlement goals.

o Collaboratively develop reasonable self-sufficiency plans and timelines with each client.

o Manage client expectations regarding resettlement services.

o Collaborate with clients, trainers, job developers, interns, volunteers, and managers in support of the client’s goals.

o Support clients by providing referrals for housing assistance, health and mental health services, public benefits, immigration legal services, identity documents, and more; advocate for clients as needed.

o Use a strengths-based approach to ensure clients are provided with the tools they need to meet their goals objectives, and assess and monitor progress, modifying initial service plans and timelines, as necessary.

o Be responsive to client inquiries, case referrals and transfers, and proactively schedule appointments with clients for service provision and follow up.

o Prepare referrals for participants needing social services.

· Administration

o Ensure compliance with both case file management and reporting requirements. Perform regular and timely maintenance of both hard-copy and electronic files per best practice standards.

o Perform various administrative tasks including data entry of intake and program forms in various online systems, internal tools, and client rosters.

o Write detailed case notes in a timely manner and maintain copies of documents and reports in case files.

o Track caseload program performance and ensure that individual program performance goals are met.

o Assist with non-OTDA/Bureau of Refugee Services (BRS) eligible cases as needed, including tracking demographic information to report to OTDA/BRS.

This is a full-time position and working hours are generally Monday through Friday with evening hours required; classes are held from 6:00pm - 9:30pm. Weekend hours may be required to meet student needs

· A bachelor’s degree in social work or a related field of study is required.

· Minimum of two (2) years of relevant work experience in human services field, preferably with immigrants or refugees.

· Strong organizational and analytical skills. Excellent communication (oral & written), and interpersonal skills.

· Highly motivated self-starter with strong problem-solving skills combined with the proven ability to multi-task, prioritize duties, and manage time effectively.

· Proficient in Microsoft Office applications (Word, Excel, Outlook, SharePoint, etc.)

· Fluent in English and Spanish

· Ability to work and communicate effectively and sensitively in a multicultural environment
