Right now we have an opportunity for a MSW Case Manager to join our team in the Emergency Department! The social worker provides professional services to patients and families, which facilitates the use of individual, public, and private resources so that optimal functioning can occur for both the patient and the family accessing care in the emergency department. These core services include assessment of psychosocial functioning and capacities, counseling for care planning and decision-making, social work treatment, and community resource planning. Mental health and chemical use evaluation, crisis interventions, traumas, social admission preventions, and abuse/neglect reporting are prioritized, and social workers will collaborate with many disciplines, programs, departments and organizations in the accomplishment of these priorities.
Required Education/Training:
- Masters Degree in Social Work preferred, or BSW considered with relevant experience.
- RN Case Manager with ED experience is also considered relevant for this position.
- Two years of experience as a Social Worker in a hospital, health organization or mental health care agency preferred.
