The Case Manager is responsible for ensuring that the highest quality support is afforded to all Day Habilitation members. The Case Manager is responsible for overseeing an assigned caseload, designing an environment that offers therapeutic and recreational activities. The Case Manager will provide hands on developmental skills training that fosters growth and independence.

Minimum Education Required:High School Diploma/GEDResponsibilities:- Collaborate with the Program Manager and Senior Case Manager to provide continuous quality improvement of individualized programs and services through the use of both internal and community resources.
- Assist with the development of program curriculum designed to promote individual growth and independence.
- Ensure that all program participant’s input to their interests and preferences have been assessed and participants are assisted in achieving their personal goals.
- Assist in the development of innovative program ideas and alternative therapeutic activities that foster individual’s empowerment and program satisfaction.
- Develop and design activity curriculums alongside the design of program schedules.
- Provide skills training and support to individuals.
- Work closely with individuals and team members to gather input for the development of creative goals.
- Responsible for the completion of daily data, monthly progress notes and semi annual reports.
- Communicate any participant needs to the Multi-Disciplinary Team review meeting.
- Ensure continuous communication between staff, participants, families, funding sources, consultants, the community, and administrative team.
- Ensure daily attendance/billing is complete and accurate.
- Ensure that all individuals’ personal care needs are met, including bathroom, meal prep and feeding and activities of daily living.
- Associate’s Degree in related field and 1 year of related experience or in lieu of a degree a minimum
- Strongly prefer that a candidate will have a demonstrated understanding of and competence in serving culturally diverse populations.
- Must have ability to read English and communicate effectively in the primary language of the program to which he/she is assigned.
- Must be able to multi-task with frequently changing priorities.
- Must possess Strong computer knowledge.
- Must have a demonstrated ability to work independently and as part of a larger resource team.
- Must hold a valid drivers’ license. Must have access to an operational and insured vehicle and be willing to use it to transport consumers.
