Case Manager, Crisis & Recovery - Sisters of Charity Health System

Under the direction of the Lead Outpatient Therapist and Clinical Director, the case manager serves as a member of the outpatient behavioral health and Rosary Hall team. The Case Manager is responsible for supporting and coordinating the mental and physical health care along with the environmental needs of an assigned patient caseload with the patient’s behavioral health and substance abuse providers. They will be responsible for conducting assessments, providing brief interventions and coordinating referrals to treatment as indicated. The individual in this position will also educate individuals and family members on available community assistance, and will use an array of evidenced-based interventions to engage individuals in care. Along with the other members of the outpatient care team, the case manager will educate medical center staff, caregivers, individuals and families on the program, and ensure the program assists individuals with accessing services internally and externally to the organization and all other duties as assigned.
