About the Job
Reporting to the Director of Health and Wellness of Los Angeles Room & Board, the Case Manager is a full-time professional staff member who provides high quality services to the residents of LARNB. The Case Manager is responsible for the health and safety of students; case management and low-level therapeutic work; program administration; data assessment & evaluation; and collaborating with internal and external stakeholders to ensure students are equipped to achieve their goals.
The Case Manager will serve the Court to College Program within the organization. The Court to College Program is a residential, higher education, juvenile justice placement for formerly incarcerated students. Each student participating in the Court to College Program is recently released from incarceration and has been ordered to continue serving their time at a step-down placement in the community. The students reside together at Opportunity House in Westwood, attend college, and must participate in mandatory programming each week. The Case Manager serves as the main point of contact for the various court-ordered programs and is responsible for creating court reports to share with Probation, the student’s attorneys, and the Court, in advance of court hearings.
The successful candidate will demonstrate an understanding of working with system-involved youth, will demonstrate a personal and professional commitment to diversity, equity, and inclusion; work with a positive young adult development centered approach that inspires, motivates, and empowers LARNB students to be successful; have excellent organizational and time management skills; demonstrate creative thinking and problem solving, and have a demonstrated record of assessment and continuous program improvement.
The Case Manager role is an annual contract, with review periods at 3 months, 6 months, and 12 months.
Required Experience:
- A minimum of two years of experience providing case management and therapeutic services
- Training in evidence-based treatment modalities
Responsibilities
- Serve as a specialist to organization staff, providing information on mental health topics and community resources for justice-involved youth.
- Support the mental health training of organization staff
- Support staff in identifying clients in need of mental health and support services
- Complete client interviews, intakes, service plans, and administer appropriate assessments
- Meet with students 1:1, at least once per week, for the purposes of building rapport, and/or assisting with linkages to behavioral health services in the community
- Meet with students 1:1 to work towards individual academic and professional goals
- Identify, refer, and connect clients and families to appropriate community resources
- Coordinate with staff and external partners to support clients’ fulfillment of court-ordered and program responsibilities
- Assist clients and families, as appropriate, in scheduling appointments with community resources
- Develop relationships with community agencies to foster an effective coordination of services for clients
- Serve as the liaison among providers involved in the care of clients
- Complete specified documentation in an accurate and timely manner and in compliance with established organizational protocol and productivity standards
- Complete mental health and biopsychosocial assessments and make recommendations for ongoing treatment and care
- Provide short-term, feedback informed, goal-focused interventions to assigned clients to assist clients in achieving goals outlined in service/treatment plans
- Utilize evidence-based treatment modalities to support the needs of all clients
- Engage in crisis intervention, as necessary
- Utilize Trauma Informed, Strength-Based, Harm Reduction and Housing First philosophies in all interactions with clients
- Maintain case notes to reflect weekly 1:1 case management meetings with students
- Complete individualized student court reports in advance of court hearings
- Attend regularly scheduled 1:1s with supervisor
- Assist with data assessment and evaluation according to an articulated plan
- Check mail, messages, and other communications regularly
- Actively participate and engage in training and professional development
- Maintain confidentiality with student information, LARNB policies and procedures, and other sensitive information one becomes privy to in the role
- Attend other meetings and perform other duties as assigned
