About the Job
Case Manager - Adults
Are you ready to make a difference in the lives of adults and their families?
Riverbend is a compassionate and dynamic mental health organization dedicated to providing high-quality services to the community. Our teams are focused, hard-working, creative people doing challenging, rewarding work! We recognize the true talents that each person brings to the team!
We are seeking compassionate and enthusiastic Case Managers to join our growing team. As a Case Manager, you will work closely with adults who experience mental illness. You’ll help to empower them to be their most independent self, and help to coordinate things in their lives that could pose challenges such as obtaining medical insurance, housing, referrals for other types of medical care, and work to build symptom management skills along the way. Our teams work to strengthen independence, improve social bonds, and reduce symptoms of mental illness.
Riverbend is looking for providers who:
- Are looking to provide 18-22 client-facing service hours per week.
- Provide community-based services primarily, but also work within our office setting.
- Are willing to provide case management, functional support services and assist with inter-agency coordination, as needed.
- Assist with crisis management, as needed, during working hours.
- Have a Bachelor’s degree in social work, psychology, or a related field, or an equivalent combination of education and experience.
- Have a passion for working with individuals and families who experience diverse challenges.
- Are life-long learners. We all learn things every day and are looking for staff who will collaborate with us, learn from each other, and bring their expertise to the table to teach and empower others.
What Riverbend offers to you:
- An opportunity to work with a diverse group of adults and families and do what you love to do based out of our Concord, NH office and greater Concord community. Remote work may be available on a limited basis as our goal is to meet our clients needs and clinical necessity.
- Group supervision available on a weekly basis.
- Individual supervision offered weekly or bi-weekly depending upon your preference and needs.
- Tons of continuing education opportunities available on a regular basis.
- And, best of all, a welcoming and inclusive culture to support your growth and development as a professional!
