Case Manager - Adult ACT Team
Riverbend Community Mental Health, Inc., one of the largest providers of community-based behavioral health services in New Hampshire, is seeking a full-time Assertive Community Treatment (ACT) Team Case Manager to join our talented multi-disciplinary team in our Community Support Program treating adults with severe and persistent mental illness.
As a Riverbend ACT Case Manager, your primary responsibilities will be working closely with clients to coordinate treatment, assist clients in obtaining the services they need and facilitate communication with Riverbend’s treatment team as well as with outside agencies. Coordination of services and ensuring the flow of communication between multiple providers is necessary. This is a great opportunity to work on the front lines of community-based behavioral health services, and help our clients achieve optimal recovery and independence.

Responsibilities
- Help clients and family members understand treatment options and make informed decisions
- Work with treatment team to facilitate case coordination and information sharing
- Participate in the development and implementation of client treatment goals
- Work with clients to build symptom management skills
- Promote effective utilization of clinical resources and other social supports
- Coordination of services between multiple medical/psychiatric providers
- Crisis intervention
Requirements
- Bachelor’s or Master’s degree (with additional responsibilities) in Psychology, Social Work, Education, Rehabilitation, or Human Services is required; or equivalent combination of education and experience.
- Excellent communication, problem-solving, time management, and organizational skills is required.
- A valid driver’s license, personal vehicle, and auto insurance is required.
- This position provides intensive services which require the employee to work one evening per week, and one to two weekend days per month.
