Job Title: Case Manager (RN)

Responsibilities:

• Develop and implement comprehensive case management plans for targeted patient populations.
• Coordinate care transitions and discharge planning to ensure continuity of care.
• Collaborate with interdisciplinary teams to assess patient needs and resources.
• Monitor patient progress and adjust care plans as necessary.
• Educate patients and families on care plans and available resources.

Qualifications:

• Current RN license in the state of Texas.
• Bachelor's degree in Nursing (BSN) preferred.
• Minimum of 2 years of clinical nursing experience.
• Strong communication and organizational skills.
• Experience in case management is a plus.
