Program Summary
The Renaissance Community, a program within Resources for Human Development (RHD) is a supportive living program serving individuals in Montgomery County. The Renaissance program offers 24-hour supervision in a community setting with an emphasis on behavioral health services. Individuals live in either a communal home environment or an apartment, based on their needs for structure and supervision. Consumers participate in activities, groups, day treatment programs and receive assistance with medication monitoring to help promote achievement toward living, learning, working and social ambition.
Position Summary
The Case Manager determines resources needed to meet the individualized needs of the participant. The Case Manager will take all necessary actions in assuring the implementation of the service treatment plan including serving as an advocate for the participant to enable him/her to receive needed services; identification of program needs, service gaps and recommending strategies. A commitment to the RHD values should be demonstrated as job duties are performed. This position's schedule will be Monday-Friday 9am-5pm, with some flexibility if needed, making $18.50 an hour.
Essential Duties and Functions
Team-Based Planning
- Act as part of a multidisciplinary team designed to provide services from a holistic and Values based perspective.
- Assist in the development of recovery plans and reviews in collaboration with the program participant, clinical and medical staff and other stakeholders as determined in collaboration with program participant.
- Develop and maintain relationships with community agencies in order to assist program participants in connecting with all available resources.
Direct Client Care
- Provide community-based services to program participants focusing on education, connection with resources, community interaction and achievement of recovery plan goals.
Administrative Functions
- Complete all assessments and documentation as required by RHD, funder, licensing/ accreditation entities.
- Other duties as assigned.
Essential Duties Addendum
- Assess and respond to needs and service eligibility of individuals in service/potential individuals in service, both in-office (approximately 60% of the time) and in the community (approximately 40% of the time) and with creative flexibility.
- Provide prompt screening and referral to appropriate treatment and supports.
- Assist with other resource/benefit acquisition and coordination as appropriate (Medicaid, Social Security, housing, social services, transportation, employment, medication assistance, etc) and develop and maintain relationships with community agencies in order to assist program participants in connecting with all available resources.
- Provide information and referrals for behavioral health and other human services, including other agencies, RHD programs, and other county agencies/departments. Collaborate with and report to these human services as necessary.
Qualifications
- Bachelor’s Degree in Human Services or related field.
- Intermediate computer skills.
- One year Case Management experience preferred.
- Two years' general work experience providing support to individuals in behavioral health, Addiction Recovery or IDD services required.
- Valid driver’s license.

- Working out in community within the service delivery area of the program. Required to drive program participants to appointments.
