Our client located in Bronx, NY is looking for a Case Manager's who's main responsibility is to provide case management services for a caseload of clients and their family members

Summary:
The Case Manager is responsible for full coordination of services for all clients assigned; ensure that all assessments are completed and all required/needed services are rendered or arranged based on service plan(s).
Job Description:
Ensure that each client is receiving appropriate and effective case management services
Conduct comprehensive assessments and develop comprehensive service plans that identify mutually agreed upon long and short-term goals.
Conduct face to face contacts (home visits/office visits) as specified in the service plan(s); with a minimum of one home visit per month and one contact per week.
Provide assistance in accessing primary medical care; assistance with adherence to medication regiments; teach activities for daily living; nutritional services; recreation programs; assistance in employment assessments, training, and private sector placement services; prevention with positives; and other necessary services to clients to increase their capacity for independent living.
Review the case management service plans and conduct reassessments on a quarterly basis to ensure that services are appropriate for the clients' current needs.
Provide crisis intervention to clients and their families as needed.
Maintain ongoing awareness of clients' medical conditions so that services are appropriate to changing needs.
Arrange support and home health care services as necessary, making referrals to medical, home care, mental health and/or substance abuse services
Ensure that clients pay the balance of their rent, telephone and utility bills.
Provide counseling and orientation regarding tenant's rights and responsibilities to maintain and retain housing.
Assist clients in moving from hospitals, nursing homes or other locations into their apartments, and, provide transportation when necessary.
In collaboration with vocational counselor and entitlement specialist provide advocacy & referrals to employment programs for training and job placement in the private sector.
Assist clients in locating alternative housing options, when a client's health so requires.
Provide and coordinate all supportive services as needed.
Collaborate with all providers within and outside of Client involved with the client/family. Coordinate and collaborate with HASA personnel as required.
Maintain regular personal contact, developing a trusting working relationship with clients in order to monitor physical and emotional capacity for independent living
Maintain and complete required documentation including all contacts made with the client and any contact on their behalf
Participate in weekly supervision
Submit a weekly schedule of field visits
Participate in mandated training workshops, seminars, and case presentations as well as meetings with peers and supervisors
Perform any other duty as assigned
Job Requirements:
Bachelor's Degree in human services or related field with two (2) years' case management work experience;Associates Degree in human services or related field with four (4) years' case management work experience; High School Diploma with six (6) years' case management work experience will be considered; Knowledge of HRA Congregate Housing Program guidelines; Direct service experience with persons with living with HIV/AIDS, substance abuse, serious mental illness, and homelessness; 5 Day/35 work week Non-Remote Position (In-Office/Some Field Work);Excellent oral and written communication skills; Proficient in Google Workspace program/Microsoft Office Suite
Extensive knowledge of case management principles and practices
Good initiative and learning ability of HRA-HASA Desk Guide and the HIV/AIDS Program
Responsible, organized, effective, and efficient. Ability to perform multiple assignments simultaneously. Communication skills (effective & Actively listening, Verbal and written)
Community minded, patient, creative, flexible, compassionate, and culturally sensitive to persons who are diagnosed with HIV/AIDS, severe and persistent mental illness.
Ability to deal tactfully and diplomatically with other Client employees, Federal, State, and City officials, professional and technical groups, clients, and the general public.
