Hope For Youth has an opening for a Case Manager in the RHYS program! Hope For Youth's Runaway & Homeless Youth Services (RHYS) provides outreach, housing and aftercare to youth under the age of 18 years old. RHYS provides youth and their families with the support and treatment needed to stabilize a youth's immediate living situation and prevent youth homelessness through the Basic Center Grant (BCG). Hope For Youth's RHYS programing can accommodate varying capacities depending on service provided. RHYS Case Manager staff work closely with the families.
How will you spend your time in this job:
- Utilizing screening assessments for safety and health concerns to inform individualized service planning
- Work with youth and their family and/or identified support network, if appropriate, to work toward reunification.
- Conducts groups focused on harm reduction, positive youth development, and life skills.
- Provide up to at least 3 months of aftercare services to youth including field work and outreach services focused on preventing homelessness/running away episodes.
- Provides support for 24-hour coverage of the RHYS crisis hotline and corresponding needs of youth, including transportation.
- Maintain records and inputting data across the continuum of RHYS including intake through aftercare.
- Conduct outreach as outlined in the grant and foster relationships with community collateral supports.
- Field work and on site work in Youth Shelter
- Documentation and related paperwork
- Staff meetings, committees, supervision, Interdisciplinary Treatment Team Meetings
- Training and staff development
- Other assignments, as needed
Minimum Requirements:
- Bachelor's Degree
- Valid NYS Driver's License in good standing
- Excellent writing skills/time management.
- Ability to be flexible with scheduling.
- Physical Requirements: read/write reports and log entries, walking/standing for prolonged periods, bending/stooping, kneeling/squatting, climbing stairs, reaching overhead, repetitive wrist/finger movements, driving
Schedule:
20 hours per week Saturday, 9am-5pm, Tuesday, 2pm-8pm and Thursday, 2pm-8pm.
