SUMMARY OVERVIEW
The Community Transition Liaison Program (CTLP): Case Assistant supports the Community Transition Liaison to operate at the top of their role and responsibility, acting as an onsite point of contact in nursing facilities (NF) for residents, families, health care proxy (HCP), and NF staff related to transitions from NF to the community. Supports and assists the Community Transition Liaison in achieving the program objectives by relieving them of various administrative and organizational details. This role is instrumental in helping the consumer to gain community support and resources for successful discharge and return to the community setting. In addition, it provides additional administrative support to the CAE program/nurses who screen consumers for clinical eligibility for community and nursing facility programs funded by MassHealth and to nurses working with consumers in the Home Care Program. This position requires a commitment to the agency's mission and goals and an investment in the success of the CTLP program.
ESSENTIAL FUNCTIONS
Essential Job functions: Duties 1 through 16 designated as ADA Essential Functions and shall be performed in this job. All other job duties are secondary functions.
- Supports the Community Transition Liaison to operate at the top of their role and responsibility
- Responsible for assigned clerical administrative functions for the Community Transition Liaison Program (CTLP)
- Accurately process, maintain and archive departmental and consumer records via manual and/or computerized means in accordance with program/system requirements
- Supports the Community Transition Liaison to assist consumers through discharge from Skilled Nursing Facilities (SNFs) back to the community
- Accompanies the Liaison on weekly Nursing Facility visits and participates in CTLP focus groups/ clinical reviews as indicated
- Ensure HIPAA compliance at all times and provides technical and logistical support for team meetings
- Maintain working knowledge of all relevant Home Care and CTLP databases, filing both electronically and in paper form.
- Gathers documentation needed to assist the consumer in applying for public benefits.
- Assists the consumer in completing and submitting housing applications.
- Ensures case documentation meets the standards set forth by the Executive Office of Elder Affairs for the Community Transition Liaison Program.
- Assists in tracking consumer activity and creating program reports for EOEA compliance and trends.
- Provides technical and logistical support for team meetings.
- Participate in agency meetings, committees and task forces.
- Attend CTLP program specific meetings, trainings and educational programs to improve skills and knowledge relevant to the job.

POSITION TYPE/EXPECTED HOURS OF WORK
This is a full-time 35hr position, Monday-Friday, 9:00 AM -5:00 PM
REQUIRED/PREFERRED EDUCATION AND EXPERIENCE:
- High School diploma or GED with 1 year of related work experience and/or interest in working with nursing facility residents including older adults or persons with disabilities who are transitioning from an institution to a community setting.
- Good computer skills including working knowledge of spreadsheets, word processing and data entry (including Windows, Word and Excel). Use table formatting in word processing and spreadsheet applications as needed
WORK AUTHORIZATION/SECURITY CLEARANCE:
- Must clear CORI background check
- Must be able to work in the United States
