SUMMARY
Assists residents with Activities of Daily Living (ADLs). Work with residents living with dementia, Alzheimer’s, and/or memory-impaired, with chemical dependency, or similar issues under the direction of the Care Manager/Director of Memory Care. May also provide emotional support and enrichment while monitoring the resident’s health and notify management of any changes in condition.
ESSENTIAL DUTIES AND RESPONSIBILITIES
- Assist residents with Activities of Daily Living (ADLs) including, but not limited to continence care, showering, grooming, transfer assistance, and meal assistance.
- Assist residents with daily recreational activities, including crafts, games, music, and exercise, and encourage participation.
- Ensure that all residents always remain clean, dignified, and presentable.
- Provide emotional support and enrichment to the residents.
- During your shift, utilize a company-provided phone to access care plans and chart care accordingly.
- Follow a care plan and document provided care accordingly.
- Read, follow, and sign any care plans and posted messages and/or updates during each shift.
- Keep any internal charts current and know of each resident’s needs.
- Observe, document, and communicate to the Care Manager or Director of Memory Care any physical, emotional, and mental changes in condition and/or all unusual observed resident behaviors (speech production, feeding patterns, toilet issues, etc.), to help facilitate assessment and development of care and service goals.
- Reports should occur by the end of the shift and should be documented in the changeover notes unless immediate intervention is needed.
- Enhance the lives of older adults by assisting them with patience and respect.
- Maintain a cooperative and cheerful attitude when relating to residents or their guests.
- Embrace teamwork and collaboration among all staff.
- Keep residents’ rooms in proper order throughout the day, including tidying rooms, making beds, and putting clothes away. Assist residents by providing for their comfort, safety & enjoyment.
- Remain up to date on all company-provided training.
- Attend periodic staff meetings, in-service training, and mandatory meetings as required.
- Observe Resident’s Rights, as posted within the facility. Promptly respond to resident calls for assistance.
- In Emergencies, assist residents according to the Emergency Plan of the facility.
- Follow proper elopement and safety protocol.
- Promotes appropriate levels of independence in resident care and demonstrates awareness of safety for both resident and self in providing care.
- Always know the location and disposition of all residents (under direct care) during shift.
QUALIFICATIONS
- Minimum of high school education or GED or combination of related experience.
- Experience is preferred, but not necessary.
- Reasonable expectations of mathematical, technical, and reasoning skills to perform essential job responsibilities.
- According to Title 22 requirements, employees must be at least eighteen (18) years of age.
COMMUNICATION SKILLS
- Ability to read and comprehend simple instructions, short correspondence, and memos in English, so as not to jeopardize the safety and well-being of the residents.
- Ability to communicate with residents, and effectively present information in one-on-one and small group situations to customers, clients, and other employees of the organization.
- Remain professional and respectful, and interact with our customers – including residents, family members, and all staff members – according to the concepts identified by the Carlton Way.
- Must be able to use technology, such as company-provided phones and tablets, to communicate among staff on resident needs.
CERTIFICATES, LICENSES, REGISTRATIONS
- Must be authorized to work in the United States of America and provide sufficient work authorization documentation(s)
- DOJ and FBI fingerprint clearance must be completed before working in the community (obtained through the community)
- Health screening/Physical and negative TB test must be less than six (6) months old (obtained through the community)
- First Aid/CPR Certification (obtained through the community)

Compensation may vary outside of this range depending on a number of factors, including a candidate’s qualifications, skills, competencies and experience, and location. Base pay is one part of the total package that is provided to compensate and recognize employees for their work, and this role may be eligible for additional discretionary bonuses/incentives.
