Care Transitions Coordinator (Business Development Specialist)
The primary purpose of your job position is to market Ashford Hall's skilled-nursing facility services to the community and referring healthcare providers, to drive referrals and admissions.
Responsibilities:
Operating within Ashford Hall's established admission and referral processes, the CTC will:
- Make frequent on-site visits, phone calls, emails, text messages, etc. to facilitate the transition of patients/residents from outside healthcare settings (hospitals, long-term acute care hospitals, behavioral hospitals, home health, hospice, and other community care settings) to Ashford Hall's Skilled Nursing Care, or Long-Term Care setting.
- Manage and facilitate information with referring data management systems (EPIC, etc.)
- Act as initial Primary Liaison with patients, patient's families, and referring healthcare professionals
- Input and record all activities and progress through Ashford Hall's CRM platform, Enquire.
- Report and communicate all incoming admissions with Ashford Hall Team.
- Attend regularly scheduled weekly meetings to report activities and progress to Management Team.
- Ensure that all activities on behalf of Ashford Hall are conducting in accordance to all applicable State and Federal rules, regulations, guidelines, and laws.
The CTC will also develop, coordinate, and direct Ashford Hall's marketing and public relations programs and services.
Other functions as may be required.
Skills:
- Proven experience as an outside marketer in the skilled nursing / long-term care industry
- In-depth knowledge of hospital discharge procedures and admission requirements for nursing homes
- Understanding of basic budgeting and financial reporting
- Knowledge of legal regulations and best practices in healthcare admission and discharges
- Great communication and people skills
- Excellent ability to work with healthcare workers in all settings and at all levels of competence
- Outstanding organizational and stress management skills
- Aptitude in problem-solving
Education:
- A Bachelor’s Degree in Nursing, Marketing, or other related field from an accredited college/university is strongly preferred, but not required.
Experience:
- Must have, as a minimum, three (3) years experience as an outside marketer working in skilled nursing/long-term care
- Must have, as a minimum, three (3) years experience working with CRM (Customer Relationship Management) and EH/MR (Electronic Health/Medical Record) systems
- Preference for Licensed Nurse will be given - either LVN or RN.
