Primary Purpose: The person in this position is a member of the Patient Centered Medical Home where each team member plays an integral role in providing patient-centered health care. The incumbent supports the care team in completing and coordinating various clerical tasks to ensure smooth and efficient patient care. |
Essential Duties: · Schedules transportation services for patients, ensuring timely and efficient transportation to and from healthcare facilities.

Other Duties: · Performs other related duties as assigned · Observes safety and security procedures; determine appropriate actions beyond guidelines; report potentially unsafe conditions; use equipment and materials properly. |
Minimum Qualification Requirements: Skills/Knowledge: · Strong communication skills, both verbal and written, with the ability to effectively interact with patients, healthcare professionals, and external service providers. · Flexibility to adapt to changing priorities and work effectively in a fast-paced environment. · Ability to maintain confidentiality and handle sensitive patient information with professionalism and discretion. Education: · High School Diploma/GED equivalent required · One year certificate from college or technical school or equivalent combination of education and experience preferred · Diploma or certificate as a Nurse aide, Medical Assistant, Emergency Medical technician, or Patient Care Technician preferred · RN with a foreign RN license or Nursing student preferred Experience: · New graduate welcome or a general experience in working in a community or hospital setting preferred · Previous experience in a clerical or administrative role, preferably in a healthcare setting.
