Job Title: Care Plan Reviewer (Clinical Case Manager - Behavioral Health)

Location: Remote (Must be an Ohio resident)
Note: Travel to Columbus, OH, for quarterly meetings/trainings is required. Some local travel (up to 50 miles) may also be needed.

What You’ll Do:
As a Care Plan Reviewer, you will help children and their families navigate health and behavioral health services. Your role will include:

• Assessing Needs: Using clinical tools to understand members’ needs and determine the best care options.
• Creating Care Plans: Developing personalized plans to connect members to the services they need, such as healthcare providers, community resources, and support programs.
• Providing Support: Offering crisis intervention and follow-up care to ensure members get appropriate help.
• Improving Care Quality: Collaborating with supervisors, medical directors, and teams to resolve challenges and achieve the best outcomes for members.
